Volume I

All participants of the November 1992 meeting, reviewers of subsequent drafts, and members of the subgroup responsible for incorporating reviewers' feedback contributed their time, input and advice to the carefully considered recommendation contained in this document.

Dr. James D. Shelton, Acting Deputy Director, Office of Population, United States Agency for International Development (USAID), has provided consistent and invaluable inspiration for the Technical Guidance Working Group (previously called the Interagency Guidelines Working Group). Ms. Jennifer Smith, Family Health International (FHI) Fellow at USAID, provided important assistance throughout the work of producing this document. Dr. Marcia Angle, Clinical Officer, University of North Carolina at Chapel Hill, Program for International Training in Health (INTRAH), served as chair of the Technical Guidance Working Group; and Dr. Douglas Huber, Medical Director, Pathfinder International, served as co-chair. Dr. Roberto Rivera, FHI Corporate Director of International Affairs, made special contributions to the development of this document, including serving on the Subgroup Responsible for Incorporating Reviewers' Feedback (see Appendix D). Dr. Carlos Huezo, Medical Director of the International Planned Parenthood Federation (IPPF), also served on the Subgroup, and offered the 1992 IPPF publication, Medical and Service Delivery Guidelines for Family Planning as a starting point for consensus-building.

The development of this document is in large measure thanks to the thorough research work of Ms. Kathryn Curtis, MSPH, and Ms. Patricia Bright, MSPH, both University of North Carolina School of Public Health Reproductive Epidemiology PhD candidates and INTRAH research assistants. Ms. Curtis and Ms. Bright methodically organized all reviewers' comments, researched the subject areas and contrasted the reviewers' comments with current epidemiologic and clinical literature, enabling the Technical Guidance Working Group to create a reference work which reflects both the concerns of the reviewers and current scientific evidence. Their thorough scholarship contributed significantly to the usefulness of this document as a reference.

This document was produced by INTRAH Technical Services staff, with the careful word processing and formatting of Ms. Barbara Wollan, INTRAH Secretary. Thorough proofreading was provided by Ms. Susan Eudy, INTRAH Administrative Secretary. Advice on instructional design and editing was provided by Ms. Catherine Murphy, INTRAH Training Materials Officer. Ms. Patricia Zook, INTRAH Training Materials Assistant, also gave careful proofreading and editorial suggestions. Helpful editing and support of this document was provided by Ms. Lynn Knauff, INTRAH Deputy Director, and Dr. James W. Lea, INTRAH Executive Director.

Volume II

This volume is a collaborative project. Volunteer Resource Persons (see table) contributed initial chapter drafts and/or assisted with subsequent revisions. Some Volunteer Resource Persons will be further revising their chapters and publishing them outside of Volume II (e.g., Cervical Cancer Prevention, Community-Based Services Checklists).

All participants of the May 1996 meeting, reviewers of subsequent drafts, and individuals responsible for incorporating reviewers' feedback contributed their time, input and advice to the carefully considered recommendations contained in this document.

Dr. James D. Shelton, Senior Medical Scientist, United States Agency for International Development (USAID), has provided consistent and invaluable inspiration for the Technical Guidance/Competence Working Group (TG/CWG). Additionally, Dr. Shelton carefully reviewed the epidemiologic interpretation for all rationales, and assured the accuracy of all recommendations. Dr. Marcia Angle, Clinical Officer, University of North Carolina at Chapel Hill, Program for International Training in Health (INTRAH) and Dr. Douglas Huber, Medical Director, Pathfinder International, served as co-chairs of the TG/CWG. Dr. Roberto Rivera, FHI Corporate Director of International Affairs, also made special contributions to the development of this document and is a member of the TG/CWG Secretariat.

The development of this document is in large measure thanks to the thorough research work of Monica Gaines, MBMA, a University of North Carolina (UNC) School of Public Health Reproductive Epidemiology PhD candidate and INTRAH Research Assistant. Ms. Gaines methodically organized all reviewers' comments, researched the subject areas and contrasted the reviewers' comments with current epidemiologic and clinical literature, enabling the Technical Guidance Competence Working Group to create a reference work which reflects both the concerns of the reviewers and current scientific evidence. Throughout the work of producing this document, important assistance was provided by Kathryn Curtis, PhD, a graduate of the University of North Carolina Epidemiology Department; Tonya Stancil, MS, a UNC Epidemiology PhD candidate and former INTRAH Research Assistant; Patricia Bright, MSPH, INTRAH Research Associate; Jill Mathis, MSH; and Jennifer Smith, MPH, Family Health International (FHI) Fellows at USAID; and Ms. Sarah Davis, a former FHI Fellow at USAID. Their thorough scholarship contributed significantly to the usefulness of this document as a reference.

This document was produced by INTRAH Technical Services staff, with the careful word processing and formatting of Ms. Betsy Rich, with assistance from Ms. Barbara Wollan, INTRAH Secretaries. Thorough proofreading was provided by Ms. Susan Eudy, INTRAH Technical Services Administrative Manager. Advice on instructional design and editing was provided by Ms. Catherine Murphy, INTRAH Senior Instructional Designer; Ms. JoElla Holman, INTRAH Director of Instructional Design and Materials Services; and Ms. Lucy Harber, INTRAH Instructional Designer. Helpful editing and support of this document was provided by Ms. Lynn Knauff, INTRAH Deputy Director, and Dr. James W. Lea, INTRAH Executive Director.
