Contraceptive methods affect
menstrual bleeding patterns in different ways. This table shows the results of multiple
studies of bleeding patterns. In general, COCs have been found to result in a cycle most
closely matched to that of a woman not using contraception. They can, however, result in
some changes in bleeding pattern, especially irregular bleeding. IUDs tend to cause
prolonged and more frequent bleeding. Finally, progestin-only methods are the most
disruptive to the menstrual cycle. Implants often cause frequent irregular and prolonged
bleeding while women using DMPA are likely to have prolonged bleeding. With all methods,
the changes in bleeding pattern lessen over time as shown by the table. Studies have shown
that bleeding problems are the most frequent reason for discontinuation of some
contraceptive methods (e.g., Implants, PICs). Counseling before method use can have a
significant impact. Women who know what to expect are better able to handle changes in
their bleeding patterns and are more likely to continue to use the method.
